Ivy Health Case Management company was established by professional case management with an excess of 20 years Case Management, Behavioral Health and Developmental Disability experience. Ivy Health Case Management services the entire state of Georgia with experienced Case Managers located throughout each of the 6 Regions.
Ivy Health Case Management is founded on providing prompt, professional, competency, advocacy, collaboration and effectiveness to members in the community. Our Case Managers demonstrate four key goals: (P.I.R.C.)
- Promote and Encourage Self-Advocacy of Health
- Improve the Quality of Care
- Reduction of Healthcare Costs; i.e. Frequent Hospitalizations
- Coordination of Care
Our objective is to provide support to decrease hospitalizations and injury, while helping to achieve safety and promote individualized best quality of life in the home and community.
Community Care Services Program
(CCSP)
The Community Care Services Program (CCSP) is a Medicaid home and community based waiver services program that provides community-based social, health and support services to eligible consumers as an alternative to placement in a nursing home.
ICWP Services
As An Eligible Member Of ICWP, The Following Services Are Provided After Choosing A Case Manager To Assist In Guiding Care:
Counseling Service
Provide members with developmental or physical disabilities in understanding and coping with their capabilities and limitations in their daily lives and or interpersonal relationships.
Adult Care
Participate in congregated community-based setting during the day, with available specialized treatment techniques for members with traumatic brain injury.
Behavior Management
Services that offer individualized interventions designed to decrease maladaptive behaviors.
Care Management
Provide members with assessing, implementing, coordinating, monitoring, evaluating options, and services required to meet the members needs and to maintain communication with active professional participants providing care to membe
Respite Care
Temporarily relief for individual(s) that normally provide care of to an ICWP member.
Skill Nurses
Treatment and healthcare procedures ordered by a Physician to provide care within the scope of nursing such as dietary practices, sanitation, and use of emergency medical services
Specialized Medical Equip/Supplies
Service that provides devices, appliances and/or materials to enable members to perceive, control and communicate within their environment.
Environmental Modifications
Services that provided physical adaptations to homes (non-rental) that are necessary to ensure the health, welfare and safety to promote and assist with greater independence.
Emergency Response Systems
24 hour, 7 days a week two-way verbal electronic communication with a central monitoring system to provide emergency access if and needed to members.
Personal Support Services
Provide personal care assistance to achieve activities of daily living; such as eating, bathing, dressing, personal hygiene, preparation of meals and light housekeeping tasks.
Consumer Directed Care Option
Personal Support Services managed by individuals to direct their own care, by hiring, training and terminating their own care providers.
Financial Support Services
Services provided to ensure consumers’ funds in the Plan of Care of managed and distributed as intended.
